Community Case Review (CCR) meets to develop and review housing attainment and stability plans for individuals experiencing chronic homelessness or at risk of chronic homelessness to identify and secure services and supports that will lead to stable housing. To expand, this group now also works to develop and review housing attainment and stability plans for All Singles who do not meet the eligibility requirements for a priority subpopulation (unaccompanied youth ages 18-24, veterans, families with minor children, and chronically homeless households and individuals).
