The Youth & Family Community Case Review (CCR) is a planning team staffed by anchor agencies within the Central Virginia Continuum of Care (CVCoC). The CCR process develops and reviews plans for youth and families experiencing homelessness or at risk of homelessness to identify and secure services and supports that will lead to stable housing. CCR team members work through the By-Name List to help problem solve difficult cases and determine prioritization for permanent supportive housing and rapid re-housing.
